What Is Menopause and What Happens to the Body as Hormones Decline?

Menopause is a natural stage of life, but the changes leading up to it can feel anything but simple.

Many women begin experiencing fatigue, poor sleep, mood changes, hot flashes, brain fog, weight changes, vaginal dryness, or a decreased interest in intimacy years before their periods completely stop. Because these symptoms can appear gradually, women may not immediately realize that changing hormones could be contributing to how they feel.

Understanding what menopause is—and what declining hormones can do throughout the body—can help you recognize your symptoms and seek the right support.

What Is Menopause?

Menopause marks the end of a woman’s menstrual cycles and reproductive years. It occurs as the ovaries gradually stop releasing eggs and begin producing significantly lower amounts of estrogen and progesterone.

Menopause is not a disease. It is a normal biological transition, although the symptoms and long-term effects can be significant for some women.

When Is Menopause Considered to Have Started?

A woman is officially considered to have reached natural menopause after she has gone 12 consecutive months without a menstrual period, as long as the absence of periods is not due to pregnancy, medication, hormonal birth control, illness, or another medical condition.

Menopause can only be confirmed after those 12 months have passed. The date of menopause is considered to be the date of the woman’s final menstrual period.

For example, if your last period occurred in July 2025 and you had no additional bleeding for 12 months, menopause would be confirmed in July 2026—but the date of your final period in July 2025 would be considered the point when menopause occurred.

The average age of natural menopause in the United States is approximately 51 to 52, although it may happen earlier or later. Natural menopause commonly occurs between ages 45 and 55.

What Is Perimenopause?

Menopause does not usually happen suddenly.

The years leading up to the final menstrual period are called perimenopause, also known as the menopausal transition. During this time, estrogen and progesterone levels may rise and fall unpredictably.

Perimenopause often begins during a woman’s 40s, but some women notice changes in their 30s. It may last several years.

Periods may become:

  • Shorter or longer

  • Heavier or lighter

  • Closer together

  • Farther apart

  • Increasingly unpredictable

A woman may skip one or more periods and then have another period months later. Each time bleeding returns, the 12-month count toward menopause starts over.

Pregnancy is still possible during perimenopause because ovulation may continue, even when menstrual cycles are irregular.

What Is Postmenopause?

Once a woman has completed 12 consecutive months without a menstrual period, she enters postmenopause.

Postmenopause refers to all the years after menopause. Some symptoms may improve over time, while others—particularly vaginal, urinary, sexual, bone, and metabolic changes—may continue or become more noticeable.

Any vaginal bleeding after menopause should be evaluated by a healthcare professional. Even light spotting or a small amount of bleeding should not be ignored.

What About Menopause After a Hysterectomy?

Determining menopause can be more complicated for women who have had a hysterectomy.

If the uterus was removed but the ovaries were left in place, periods stop because there is no uterus, but the ovaries may continue producing hormones until natural menopause occurs. Because there is no menstrual cycle to follow, symptoms, age, health history, and occasionally laboratory testing may help determine whether menopause has occurred.

If both ovaries are surgically removed, the sudden loss of ovarian hormones causes surgical menopause. Symptoms may begin more abruptly than they do during natural menopause.

Certain medications, chemotherapy, radiation, and medical conditions may also cause menopause or ovarian insufficiency earlier than expected.

Do Women Have “No Hormones” After Menopause?

Women do not completely lose all hormones after menopause.

The adrenal glands, body fat, and other tissues continue producing or converting small amounts of hormones. However, ovarian production of estrogen and progesterone falls substantially. Testosterone and other androgen levels may also decline gradually with age.

These hormones do much more than regulate menstrual periods. Hormone receptors are located throughout the body, including in the brain, bones, skin, blood vessels, breasts, muscles, vagina, bladder, and reproductive organs.

That is why declining hormone levels can affect many different areas of health.

What Happens to the Body as Estrogen Declines?

Hot Flashes and Night Sweats

Declining and fluctuating estrogen can affect the brain’s temperature-control center.

A small change in body temperature may suddenly trigger:

  • Intense warmth

  • Facial or chest flushing

  • Sweating

  • Chills afterward

  • A rapid heartbeat

  • Waking during the night drenched in sweat

Hot flashes are one of the most common symptoms of the menopausal transition and may continue for several years.

Sleep Disturbances

Sleep may become lighter or more interrupted during perimenopause and menopause.

Night sweats can cause repeated awakenings, but some women experience insomnia even without hot flashes. Poor sleep can then contribute to fatigue, irritability, anxiety, food cravings, difficulty concentrating, and decreased motivation.

Brain Fog and Difficulty Concentrating

Many women describe:

  • Forgetting words or names

  • Walking into a room and forgetting why

  • Losing their train of thought

  • Difficulty multitasking

  • Feeling mentally slower

  • Trouble focusing at work

Hormonal changes, disrupted sleep, stress, mood symptoms, thyroid conditions, medications, and other health concerns may all contribute. Brain fog should not automatically be blamed on menopause without considering other possible causes.

Mood Changes

Hormone fluctuations may affect neurotransmitters and areas of the brain involved in mood regulation.

Some women experience:

  • Irritability

  • Anxiety

  • Tearfulness

  • Feeling overwhelmed

  • Lower stress tolerance

  • Mood swings

  • Symptoms of depression

Women with a previous history of anxiety, depression, severe premenstrual symptoms, or postpartum mood changes may be especially sensitive to hormonal fluctuations. Approximately four in ten women experience mood symptoms during perimenopause that resemble premenstrual symptoms.

Persistent depression, severe anxiety, or thoughts of self-harm require prompt professional care and should never be assumed to be “just hormones.”

Vaginal and Vulvar Changes

Estrogen helps maintain the thickness, elasticity, blood flow, moisture, and healthy tissue of the vagina and vulva.

As estrogen declines, some women develop:

  • Vaginal dryness

  • Burning or irritation

  • Itching

  • Pain with sexual activity

  • Light bleeding after sex

  • A feeling of tightness

  • Decreased sensation

These changes are often part of genitourinary syndrome of menopause, sometimes called GSM. Unlike hot flashes, vaginal and urinary symptoms often do not resolve on their own and may worsen without treatment.

Low estrogen can cause the vaginal walls to become thinner, drier, less elastic, and more easily irritated.

Urinary and Bladder Changes

Declining estrogen can also affect the urethra, bladder, pelvic tissues, and urinary tract.

Women may notice:

  • Urinary urgency

  • Increased frequency

  • Burning with urination

  • Bladder discomfort

  • Leakage

  • Recurrent urinary tract infections

These symptoms should be evaluated because infection, pelvic-floor dysfunction, diabetes, bladder disorders, and other conditions may cause similar problems.

Changes in Sexual Health

Sexual desire can be influenced by estrogen, testosterone, sleep, stress, relationship factors, medications, mood, body image, vaginal discomfort, and overall health.

During and after menopause, some women experience:

  • Reduced sexual interest

  • Difficulty becoming aroused

  • Less sensation

  • Difficulty reaching orgasm

  • Pain during intercourse

  • Avoidance of intimacy because of discomfort

Sexual concerns are real health concerns and deserve to be discussed without embarrassment.

Bone Loss

Estrogen helps protect bone by maintaining a healthy balance between bone formation and bone breakdown.

When estrogen declines, bone loss accelerates. Over time, this may increase the risk of:

  • Osteopenia

  • Osteoporosis

  • Loss of height

  • Spinal compression fractures

  • Hip, wrist, and other fractures

Osteoporosis is sometimes called a “silent” condition because bone loss may occur without symptoms until a fracture happens. Women are at increased risk after menopause because estrogen plays an important role in maintaining bone strength.

Adequate calcium and vitamin D, resistance training, weight-bearing activity, avoiding smoking, limiting alcohol, and receiving appropriate bone-density screening can help protect bone health.

Changes in Cholesterol, Blood Vessels, and Heart Health

Estrogen affects blood vessels, cholesterol metabolism, insulin sensitivity, and the distribution of body fat.

After menopause, women may experience changes such as:

  • Higher LDL cholesterol

  • Changes in blood pressure

  • Greater insulin resistance

  • Increased abdominal fat

  • Reduced blood-vessel flexibility

Cardiovascular risk rises with age, but menopause is only one part of that picture. Genetics, smoking, diabetes, kidney disease, high blood pressure, cholesterol, physical activity, sleep, nutrition, and body composition also matter.

Hormone therapy should not be started solely to prevent heart disease. Its risks and benefits must be considered individually.

Muscle, Joint, and Body-Composition Changes

Aging and hormonal changes can contribute to:

  • Loss of muscle mass

  • Reduced strength

  • Increased abdominal fat

  • Changes in metabolism

  • Joint stiffness or discomfort

  • Longer recovery after activity

Menopause does not make weight gain inevitable. However, reduced muscle mass, poor sleep, stress, insulin resistance, aging, and lifestyle changes can make maintaining weight and body composition more difficult.

Strength training, adequate protein, regular movement, restorative sleep, and evaluation for conditions such as thyroid dysfunction can be important.

Skin and Hair Changes

Estrogen contributes to skin hydration, collagen production, thickness, and elasticity.

As estrogen declines, women may notice:

  • Drier or thinner skin

  • Increased wrinkles

  • Reduced firmness

  • Slower wound healing

  • Hair thinning

  • Changes in hair texture

Hormonal changes during menopause can contribute to thinner and drier skin, although sun exposure, genetics, nutrition, smoking, thyroid disease, iron status, stress, and aging also play important roles.


Could Most Women Benefit From Hormone Therapy?

For many women, the answer may be yes.

Estrogen, progesterone, and testosterone do much more than control menstrual periods. Hormone receptors are found throughout the brain, bones, muscles, skin, blood vessels, breasts, vagina, bladder, and other tissues. As ovarian hormone production declines, women may experience changes in temperature regulation, sleep, mood, cognition, sexual health, vaginal and urinary tissues, bone density, muscle mass, body composition, and overall quality of life.

Hormone therapy can replace some of what the body no longer produces. It is the most effective treatment for menopausal hot flashes and night sweats, can improve many vaginal and urinary symptoms, and helps prevent the bone loss associated with menopause. For many healthy women who begin treatment before age 60 or within approximately 10 years of menopause, the benefits may outweigh the potential risks.

Because hormone loss affects the entire body, every woman deserves an informed conversation about hormone therapy even if she does not have severe hot flashes. Women should be given the opportunity to understand what is changing in their bodies, the potential benefits of replacing hormones, the possible risks, and the different treatment options available.

This does not mean that the same hormone plan is appropriate for every woman. Treatment must be individualized according to:

  • Her age and time since menopause

  • Her symptoms and health goals

  • Whether she still has a uterus

  • Her personal and family medical history

  • Her bone, breast, cardiovascular, and metabolic health

  • The hormone, dose, and method of delivery being considered

  • Her individual preferences and comfort level

Some women may not be appropriate candidates for systemic hormone therapy. This can include women with certain hormone-sensitive cancers, unexplained vaginal bleeding, active or previous blood clots, stroke, heart attack, significant liver disease, or other medical concerns. A cancer diagnosis does not always mean that every type of hormone treatment is permanently prohibited, but it does require careful evaluation and often collaboration with the woman’s oncology or specialty team.

Women who cannot use systemic hormone therapy may still have other options. Depending on their health history and symptoms, these may include nonhormonal medications, localized vaginal therapies, pelvic-floor treatment, bone-protection strategies, lifestyle support, and other individualized approaches.
